Rafal Lachcik Death – In the early hours of Friday, June 16, 2023, the Dudley Police Department and the Dudley Fire Department and Emergency Medical Services were called to the vicinity of Schofield Avenue near Perryville Road in response to a report of a collision involving many vehicles. The Webster Police Department was also dispatched to the scene.
Officers found a severely damaged vehicle with a single occupant when they arrived at the scene. Until emergency medical services arrived, the officers made attempts to provide lifesaving aid. The male driver, who was 28 years old, was taken to UMass Hubbard for treatment, but he was unable to recover from his injuries and passed away there. Loved ones have taken to social media to identify him as Rafal Lachcik of Thompson, Connecticut.

Three individuals in their 20s and one person aged 18 were killed in a crash in Thompson on Monday afternoon, according to police. Three of the four deceased individuals are from Providence, Rhode Island. The Honda Civic, piloted by 18-year-old Johan Santana, was traveling south on Quaddick Town Farm Road at 5 p.m., according to the state police. There were four other passengers onboard.
According to state police, Santana lost control of the Honda while attempting to pass another southbound vehicle. According to police, the vehicle drove off the right side of the road and struck a tree. Santana and Jaslin Perez, both 21 years old, were pronounced dead at the site.
Erika Perez, 21, and Lizandra Terrero Almonte, 21, were rushed to UMass Hospital but subsequently died from their injuries, according to police. In addition to Jaslin Perez and Santana, Terrero was from Providence. Police have not yet specified where Erika Perez is from. The condition of the fifth passenger is listed as critical.
There were no other injuries reported. The incident is being investigated by state police.
